Job Description

The Johnson County Community Developmental Disabilities Organization (CDDO) is the single point of entry for IDD services in Johnson County and part of Johnson County Developmental Services. JCDS serves people with intellectual and developmental disabilities.  We focus on individuals’ abilities, provide choice driven supports, and advocate alongside people to live and work in our community.  We aspire to build a community that promotes, supports, and embraces people of all abilities.  As a provision of the Developmental Disabilities Reform Act, a CDDO has been designated for every area of the state to help coordinate services and supports for individuals with intellectual and developmental disabilities (I/DD). We are searching for a Functional Assessment Assessor.  First review of applications for this position will begin on October 24, 2024. 

 

Responsibilities include:

  • Maintains monthly assessment caseload and ensures that caseloads are evenly distributed among Assessment Team; works with CFO to do monthly scheduling pulls from the state system.
  • Reviews CDDO records and verifies personal information, medical information, ensures functional assessment standard, regulations, and processes re being followed. 
  • Coordinates and schedules annual Assessments with targeted case managers; makes telephone calls/emails and schedules Functional Assessments; accommodates schedules in order to ensure opportunity for participation; provides information and guidance to ensure that individuals, families and providers are aware of requirements for the Functional Assessment;  responds to requests and performs special requests functional assessments for courtesy requests from other CDDO areas, crisis and exception, waiver transfers, ICF-IID facilities, etc.
  • Facilitates Assessment meetings, ensures equal participation from attendees, builds consensus, and maintains positive focus to meetings; maintains compliance with HIPAA guidelines and maintains files in accordance  with Federal, State and County policies; administers functional assessment tool and skills worksheets in accordance with state policy and guidelines; submits assessment to the State of Kansas for scoring; ensures that supporting documentation is gathered and retained; provides comprehensive options counseling to individuals and families.
  • Provides information related to Functional Assessments in accordance with HIPAA guidelines to interested parties; testifies at court hearings regarding the completion of Functional Assessments as required; assists with Requests for Review or Appeal; addresses issues from individuals, families, or providers related to the Functional Assessment.
  • Attends and participates in CDDO team meetings and events and statewide workgroups; represents the CDDO at local and state meetings; serves as main point of contact for Functional Assessment questions. provides training and technical assistance as needed; participates in continuous quality improvement efforts and recommends best practices; ensures CDDO publications and forms are updated.
  • Provides input on processes and procedures related to Functional Assessments; develops processes to ensure compliance with State. Complete annual Functional Assessment training from the state.

Job Requirements

High school and two (2) years experience in IDD or related field.

 

Bachelor’s degree in Human Services, Education, Rehabilitation or related field and six months of experience in the field of developmental disabilities are preferred. 

 

Must pass the state functional assessment training and obtain completion certificate within 60 days of employment and annually thereafter.

 

Experience may substitute for education. 

 

Candidate must have a valid driver's license and able to successfully complete criminal and motor vehicle background screenings, which includes KDADS required screenings and registry checks.
