Job Description

The Health Supports Specialist (LPN) provides a variety of skilled nursing tasks under the supervision of a Registered Nurse.  Duties included are:

 

Conducting quarterly nursing assessments, provide emergency medical care, assess for illness, monitor chronic disease processes, and administer first aid. Give Injections, assess skin breakdown and provide wound care. Teaching staff how to perform delegated tasks in regard to catheters, ostomies, feeding tubes, O2, CPAP, suction and nebulizer use and performing those tasks at times also.  

 

Teach proper Medication Administration to unlicensed staff, along with legal charting, the use of Electronic Health Record and Electronic Medication Record and the pharmacy messaging and ordering sites.  Provide education on infection control and bloodborne pathogens. Participate in multi-disciplinary teams to assess, develop and provide enhancement to promote healthy lifestyles. Help to track the use of DME for all residential individuals and find providers to get preferred supplies, maintain adequate stock. Facilitate on-site clinics, scheduling, sending reminders and getting post appt. notes to maintain medical records.

 

Some travel will be required to perform site visits and verify proper medication administration procedure, ensure organized medical supplies, storage and disposal medication and complete assessments.  The Health Supports Specialist will support staff to learn the proper techniques in personal care and demonstrate physical positioning of individuals, to reduce pressure and maintain skin integrity and comfort.  They may help individuals to set up medical appointments with providers and accompany them as needed. Help guide them in making decisions toward their best possible health and ensure all medical referrals and recommendations are getting completed. 

 

Serve in a regular on-call rotation to ensure 24-hour nursing services and facilitate ER and urgent care visits, meet individuals at the hospital after ambulance transfers to set up clear communication between facilities and provide complete and precise health history and communicate back to the circle of support.  Other duties as assigned.

Job Requirements

Minimum Job Requirements:

High school diploma or equivalent and graduation from an accredited Practical Nursing program are required.  Kansas Licensed Practical Nurse (LPN) license is required Registered Nurse (RN) license is applicable.  May require KS/MO driver’s license, depending on department or agency.

Johnson County Government requires reference/background screening for all positions.  

 

Preferred Job Requirements:

One year of nursing experience is preferred.

 

Physical, Environmental, and Special Working Conditions:

Occasional need for excessive standing and/or walking.

Lifting: varies by assignment.

Exposure to hazardous materials (bodily fluids).

All County employees may be called upon to assist other departments in a declared emergency situation.
