We’re seeking a compassionate and skilled Health Supports Specialist (LPN or RN) to join our dedicated nursing team. In this role, you’ll provide direct nursing care, support individuals with complex health needs, and play a vital role in educating and supporting unlicensed staff who provide daily care. —all while helping people live healthier, more independent lives.

Job Description

  • Deliver skilled nursing services in individuals’ homes, including:
    • Monitoring chronic conditions
    • Assessing for any skin breakdown and other concerns and providing wound care and guidance when needed.
  • Respond to signs of illness or injury, and facilitate care when advanced medical evaluation is needed
  • Accompany individuals to emergency or scheduled medical appointments as needed
  • Serve as a clinical resource for unlicensed staff, offering:
    • Guidance on safe and accurate Medication Administration
    • Support with delegated health tasks (e.g., catheter care, tube feeding, suctioning, nebulizer use, CPAP/O2)
    • Instruction on infection control and bloodborne pathogen safety
  • Teach and support use of Electronic Health Records (EHR), legal documentation, and pharmacy communication systems
  • Participate in interdisciplinary care teams to develop and enhance individualized health plans
  • Help to organize on-site clinics and maintain accurate medical records
  • Conduct site visits to ensure proper medication storage, administration practices, and overall health and safety
  • Help coordinate medical appointments, follow-ups, and ensure medical recommendations are carried out

On-Call Responsibilities:

  • Respond to urgent situations, including ER and urgent care visits
  • Provide staff with timely medical advice, support, and real-time medical guidance for the people we serve.
  • Help ensure continuity of care by facilitating communication between medical providers, staff, and individuals served

Job Requirements

Minimum Job Requirements:

Highschool diploma or GED

Active LPN or RN license in the State of Kansas

Strong clinical assessment skills

Valid driver’s license

Excellent communication, both written and verbal

Comfort using electronic documentation systems (EHR)

Experience training unlicensed staff

Passion for person-centered care and community health

 

Preferred Job Requirements:

One (1) year of nursing experience is preferred.

Experience working with individuals with developmental disabilities

 

Physical, Environmental, and Special Working Conditions:

Occasional need for excessive standing and/or walking.

Lifting: varies by assignment.

Exposure to hazardous materials (bodily fluids).

All County employees may be called upon to assist other departments in a declared emergency situation.
