Johnson County Mental Health is looking for qualified candidates to fill a Children's Case Manager position at our Mission and Olathe Mental Health office.  Case Managers work as part of a treatment team in the Pediatric Services program, providing services to support children, ages 4-18, with Severe Emotional Disabilities (SED).  Evening availability is required and the schedule will vary based on the needs of the children served.  This position will remain open until filled.   - Conducts assessments of individuals' background and applicable circumstances to identify needs and/or determine service eligibility; interprets and communicates relevant information to individuals, families, and other stakeholders. - Develops plans to address the needs of individuals in single or multiple areas of focus; networks with other community agencies to research and coordinate a variety of resources and services to address short term intensive or long term individual needs; encourages the establishment and maintenance of support systems; consults and collaborates with other providers in the development, implementation and review of plans. - Provides a variety of services for a designated case load of individuals with multi-faceted and/or ongoing needs; ensures services provided are consistent with individual plans; initiates and attends individual and/or group meetings and provides services in a community based setting; provides transportation of individuals according to the needs identified in plans; organizes and facilitates skill development and support groups. - Completes and submits documentation regarding services provided; evaluates and documents individuals' progress according to measurable goals; reviews quality and effectiveness of services provided.

Job duties includes: - Assesses psychiatric symptoms, psychosocial issues, likely side effects from medications, potential medical problems, mental status, and possible need for change in treatment regimen through in-person (R.N. Medication Reviews) or phone contact with patients/collateral historians; consults with the psychiatrist in-person or by phone; verifies psychiatric provider orders; explains any ordered changes to patient/family; coordinates changes in treatment with other providers/pertinent personnel (e.g. case managers, pharmacists, Jail Health services staff, residential substance dependence program staff, etc.); monitors response and gives feedback as indicated; administers intramuscular injections as ordered by the psychiatric provider; identifies and coordinates the patient's clinical needs with other members of the treatment team (e.g. therapists, case managers); tracks and makes available for the psychiatrist necessary lab and other investigative results that have been complete; administers and tracks clinically necessary rating scales (e.g. AIMS to monitor for development of tardive dyskinesia), and refer to the psychiatric provider to evaluate.  - Supervises the systematic and efficient work flow for nurses within the clinic on a day-to-day basis including coordination of nursing coverage; delegates assignments and provides direction for the nursing staff in accordance with their level of training and the patients acuity; works in cooperation with medical providers and other MNH staff to ensure the best quality of patient care; provides quality review of documentation and general psychiatric nursing practice of nurses; responsible for monitoring of adherence to policies set forth by NCCHC to maintain accreditation; works with leadership and the providers to develop the weekly schedule, attends the monthly meetings, and coordinate with outside providers.   - Consults with other MHC staff regarding clinical and medication issues; discusses medical/psychiatric/medication concerns as noted by other staff (e.g. therapists, case managers, correctional facility personnel) - Provides education to patients/families/others; accesses, maintains, and makes available educational materials; educates patients and families directly and by phone about psychiatric illnesses, medication expectations and potential side effects, basic wellness and medical issues; develops and provides mental health and medication education to others both informally and formally.

The Crisis Clinician will have two primary roles on the team.  First, the Crisis Clinician will be paired with a MCRT Crisis Case Manager to provide mobile crisis response (see work schedule for current availability). The pair will be dispatched by JCMHC’s 988 Crisis Line team to safely respond to behavioral health crises occurring in the community. Safety is critical in these roles and the pair will request law enforcement support at their discretion. The pair will de-escalate, assess and support individuals through their crisis and complete timely follow up, either by phone or at their home, to ensure the individual connects to appropriate resources and/or the crisis has resolved. The second role of the Crisis Clinician is to provide ongoing clinical support for clients on MCRT case managers’ caseloads. This includes outreach visits with MCRT case managers to high acuity clients or those with complex circumstances for the purposes of clinical assessment, safety planning, and crisis stabilization. Other duties includes: - Responding across Johnson County as part of a 2-person team, providing mobile crisis interventions to a variety of individuals in crisis. - Assessing risk and develop thorough safety and/or crisis stabilization plans, referring to higher level of care when needed and maintaining client safely in the community when possible. - Completing state hospitalization screening and collaborating with law enforcement on emergency detention as appropriate relating to the crisis. - Providing support, education, and connection to resources, as needed for appropriate crisis response. - Providing proactive outreach to prevent mental health crisis and/or criminal justice recidivism. - Collaborating with JCMHC teams, law enforcement, family members, hospitals and other community agencies to advocate and partner for individuals’ needs. - Completing and submitting clinical documentation regarding services provided.
